Hospital Medicine

As a member of the Columbus Community Hospital health care team, hospitalists work with your primary care doctor, surgeons, specialists, nurses, radiologists, therapists, pharmacists and others involved in your care.

The hospitalist gives your primary care doctor updates on any changes in your condition during your hospitalization. The hospitalist makes dismissal arrangements for you including prescribing take home medications you may need. When you leave the hospital, your hospitalist calls your primary care doctor and sends a summary report of your care. Your primary care doctor provides your follow-up care after you go home. If you do not have a doctor, you will be referred to one.

Work with you, other hospital staff members and physicians to coordinate your care.

Call your primary physician to review your clinical course upon dismissal.

Provide frequent follow-up visits during your stay and will be available to discuss treatment plans and answer questions.

Fax your doctor a summary discharge report on dismissal.

Notify your primary physician of any unexpected changes in your conditions to make timely decisions.

Assist you in getting in touch with your doctor to assist with follow-up care.
